Postural neck pain is the low-grade ache across the neck and upper shoulders that builds through a working day and has largely settled by the next morning. It is not a sign of damage. It is a loading problem — the neck being asked to hold one position for longer than it currently has the endurance for. Fixing it means changing the demand and building the endurance, in that order.

What causes it

  • Long uninterrupted stretches at a screen without changing position
  • A monitor set too low, which pulls the head forward and down
  • Laptop use without a separate keyboard or a stand
  • Phone and tablet use with the head held down for long periods
  • Low endurance in the deep neck and shoulder blade muscles
  • Stress, which quietly raises resting tension through the upper shoulders

What you might notice

  • Ache across the top of the shoulders and base of the neck, worst late in the day
  • Clearly tied to the working week — better on weekends and holidays
  • Tightness or a weight rather than sharp pain
  • Tension headaches building towards the end of the week
  • Stretching helps, but the relief only lasts a short while

How we treat it

  • A workstation review — screen height, chair, desk and keyboard position
  • Movement breaks built into the day at a frequency you will actually keep to
  • Deep neck flexor and lower trapezius endurance work, which is what holds the change
  • Manual therapy and dry needling for relief while that endurance is being built
  • Upper back mobility work, since a stiff thoracic spine forces the neck to compensate
  • Practical strategies for laptop and phone use outside work hours
